The first time I was driving about 30 miles an hour. There was a car in front of me about 10 car lengths who put on his blinker to make a left turn. I started to tap my brakes to slow down. The car was so far in front of me that I didn’t even need to slow down, I was being careful. Suddenly the truck shuddered and stopped and BRAKE, BRAKE flashed on the screen in front of the steering wheel. Everything in the truck flew forward. The people in the vehicle behind me started blasting the horn because they almost rear ended me. My truck just slammed on the brakes for no reason. Another time I was driving down the interstate highway. There were no cars in front, behind or on either side of me. Suddenly, the truck shuddered and BRAKE, BRAKE flashed across the screen. I was driving about 70 miles an hour. The truck did not stop. It just shuddered and slowed down. Those were the two scariest times. The truck since then will flash BRAKE on occasion if I step on the brakes to start slowing down. I called customer service for American Honda and they told me to take it to a Honda dealer when it was happening.

After I braked to slow down, I began to accelerate as the vehicle in front continued forward. As soon as I hit the accelerator, the Automatic Emergency Braking kicked in and locked up the brakes - even though the car in front of me was 20-30+ feet away. Lost brakes as I was trying to decelerate to make a left turn on FM 1472, still managed to come to a stop before hitting the traffic ahead of me that was also waiting to make a left turn. Smoke started to come out from the wheel wells. Put vehicle on parking, shut off engine, opened hood and found fire coming out from the back of the engine bay. Extinguished fire with a Fire extinguisher, and a few minutes later, a Fire Dept Truck arrived to hose it just to make sure it was properly extinguished. Waiting for Cause and Origin investigator from the insurance to inspect the vehicle.

Consequence: Corrosion of the wiring harness can cause increased electrical resistance, unexpectedly activating the vehicle stability assist (VSA) system, increasing the risk of a crash.

The Office of Defects Investigation (ODI) has received 414 reports of connecting rod bearing failures in the 3.5L V6 engine used in the following vehicles: Model Years (MY) 2018-2020 Acura TLX, MY2016-2020 Acura MDX, MY2016-2020 Honda Pilot, MY2018-2020 Honda Odyssey, and MY2017-2019 Honda Ridgeline. The subject engine failures were analyzed as part of a Recall Query (RQ24013). It was determined that these engine failures are outside the scope of Honda Recall 23V-751. Further, evidence gathered in conjunction with RQ24013 does not suggest this failure is caused by the same crankshaft manufacturing defect that is addressed by Honda in Recall 23V-751. Based on analysis of data from RQ24013, NHTSA has closed that investigation. The significant number of reports of engine failure in vehicles outside the scope of RQ24013 presents a potential safety risk that warrants further investigation. ODI is opening this Preliminary Evaluation (PE) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues. On June 26, 2024, the Oﬃce of Defects Investigation (ODI) opened a Recall Query (RQ24011) to assess the long-term suitability of the remedy for recall 22V-867, which was ﬁled on November 23, 2022 by American Honda Motor Co. (Honda). Recall 22V-867 addressed concerns of a loss of the Rear-View Camera (RVC) function in model year (MY) 2017-2019 Honda Ridgeline vehicles. Recall 22V-867 states that the RVC wire harness was manufactured with a protective corrugated tubing which was insufficient to protect against bending wear and zip ties which were insufficiently tightened and would not keep the protective tubing in place. The consequence is that the RVC wire harness may fatigue and break after repeated opening and closing of the tailgate. This ultimately results in the failure of the RVC image to display due to a lack of connectivity. Honda’s remedy identified in the recall filing was a replacement harness with longer protective corrugated tubing and sufficiently tightened zip ties. This remedy part was also installed as a production part in certain MY 2019 Ridgeline vehicles. On May 3, 2024, Honda submitted recall 24V-321 to ODI, addressing concerns of a loss of the RVC function in MY 2020-2024 Honda Ridgeline vehicles. Recall 24V-321 states that the RVC wire harness was manufactured with a material which was susceptible to breakage upon repeated opening and closing of the tailgate, which ultimately leads to a complete loss of RVC function. On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation. NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.

On February 7, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22-001) to assess alleged failures of the rear-view camera (RVC) in 2017-2018 Model Year Honda Ridgeline vehicles. At this time, ODI has received a total of 107 complaints with allegations of the RVC image failing to display when the vehicle was in reverse.The RVC wire harness is manufactured with a protective corrugated tubing and harness guide to help mitigate stress on the RVC wire harness when the tailgate was opened and closed. Honda determined the length of corrugated tubing inserted into the harness guide was insufficient to provide optimal protection for the wire harness. This lack of protection for RVC wire harness led to severe wire harness damage that ultimately led to the failure of the RVC image to display due to a lack of connectivity.On November 23, 2022, Honda filed a defect information report recalling all 2017-2019MY Honda Ridgeline vehicles. The dealer will replace the RVC tailgate harness with an improved wire harness that has longer corrugated tubing to protect the wire harness.In view of the recall action being taken by Honda, 22V-867, ODI is closing this Preliminary Evaluation.
