The contact stated that while driving at an undisclosed speed and approaching a traffic light, the brake pedal was depressed; however, the engine surged. There were no warning lights illuminated. The vehicle was taken to the local dealer on several occasions, who was unable to determine the cause of the failure. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The failure mileage was approximately 35,000.

The contact owns a 2022 Toyota Highlander Hybrid. The contact stated while driving approximately 20 MPH and slowing down for a traffic signal, the contact felt as if the brakes had failed. The contact stated that while depressing the brake pedal, the vehicle was still rolling forward. The contact stated that he depressed the brake pedal with more force and eventually the brakes grabbed and stopped the vehicle. The contact stated no warning light was illuminated. The contact had taken the vehicle to a local dealer, but the failure could not be duplicated. The contact stated that the failure had become a recurring failure. The vehicle had not been repaired. The manufacturer had not been informed of the failure. The failure mileage was approximately 20,000.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Highlander and Highlander Hybrid vehicles. The second-row seat backs may fail to lock into position during seat back adjustment.
Consequence & remedy
Consequence: A seat back that fails to lock may not properly restrain an occupant during a crash, increasing the risk of injury.
Remedy: Dealers will replace the return springs in the seat back recliner assemblies, free of charge. Owner letters were mailed September 9, 2026. This is a phased recall. Owners may contact Toyota’s customer service at 1-800-331-4331. Toyota's numbers for this recall are 26TB06 and 26TA06.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2020-2023 Highlander & Highlander Hybrid vehicles. During normal vehicle operation, minor impact to the front lower bumper cover may result in the cover coming loose or detaching.
Consequence & remedy
Consequence: A detached front bumper cover can become a road hazard, increasing the risk of a crash.
Remedy: Dealers will repair or replace the upper and lower front bumper covers as necessary, free of charge. Owner letters were mailed April 22, 2024. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 23TB12/23TA12.
Gulf States Toyota, Inc. (GST) is recalling certain 2022 4Runner, Tacoma, Highlander, and Highlander Hybrid vehicles. The load carrying capacity modification labels may not be permanent and can fade, becoming illegible. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 110, "Tire Selection and Rims."
Consequence & remedy
Consequence: An illegible label may allow the vehicle to be overloaded, increasing the risk of a crash.
Remedy: GST will notify owners by mail, including a replacement load carrying capacity modification label for their vehicle and detailed replacement instructions, free of charge. Owner notification letters were mailed June 30, 2022. Owners may contact GST customer service at 1-800-444-1074. GST's number for this recall is 22R1.
Toyota Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus LX 600, Lexus NX 350h, Lexus NX 450h+, Toytoa RAV4 Hybrid, 2021-2022 Lexus LS 500h, Toyota Mirai, RAV4 PRIME, Sienna, Venza, and 2020-2022 Toyota Highlander Hybrid vehicles. The Skid Control ECU software may not turn on the Vehicle Stability Control (VSC) system when the vehicle is restarted after the driver has disabled the VSC system. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."
Consequence & remedy
Consequence: Driving with a deactivated stability control system increases the risk of a crash.
Remedy: Dealers will update the Skid Control ECU software, free of charge. Owner notification letters were mailed May 27, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 22TA03. Lexus' number for this recall is 22LA01.

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
