I am reporting a severe, systemic vehicle performance discrepancy on my 2025 Lexus TX 350h (2.4L Turbo Hybrid powertrain). The vehicle is officially labeled with an EPA rating of 23 MPG combined. However, under normal, conservative driving conditions, the vehicle routinely achieves only 17-17.7 MPG—a consistent 26%+ shortfall from the manufacturer's certified window sticker metrics. I have never gotten EPA higher than 17.7 since owning the new car. This has drastically limited the vehicle's safe total driving range to under 300 miles per tank. The shortfall persists despite cautious driving in Eco Mode and after passing the engine break-in period. This issue matches a widespread pattern reported by numerous other owners in consumer forums, pointing to a potential defect in the vehicle’s fuel management software or hybrid battery management logic. I request that NHTSA log this tracking data to investigate whether the manufacturer misrepresented the vehicle's real-world operational capabilities or fuel-system integration. I have a separate compliant #11762922 open for extreme safety concern related to visible gas vapors and overpowering health concerning gasoline smell at times which have occurred 5 times that I have recorded; unsure if this issue is related. Please investigate all the Lexus TX and Toyota Grand Highlander formal NHTSA complaints, Lexus Tickets, Technical Service Bulletin requests, and the previous Lexus Service repair for fuel leaking being a fire hazard - the fix was a fuel tank protector under Service Campaign #25LC05. My car had this protector installed; ironically there were no incidents of the gas smell & visible vapors until AFTER the service campaign and install of the fuel protector; coincidence or not. It seems highly probably that the following 4 issues are related on these cars: 1) fuel leaking/protector installed 2) gas mileage off 3) Visible gas vapors 4) strong gas smell. Please act before someone gets hurt. This is a family car w kids.

I am reporting a recurring potential fuel-system/EVAP safety issue with a 2025 Lexus TX 350 (same as Toyota Grand Highlander). On FIVE separate occasions (May, 6/27, July, 8/14, and 9/8/2026), I experienced an overwhelming raw gasoline odor associated with visible vapors coming from the fuel-filler/gas-cap area. Three incidents occurred after refueling; the two most recent occurred WITHOUT recent refueling and with less than half tank full. On 8/14, with temps in the 90s and approximately 135 miles to empty, I drove 30–40 miles, idled approximately 45 minutes, then drove another 10 miles. The overwhelming gasoline odor and visible vapors occurred again. On 9/8, at approximately 108°F and with 152 miles to empty, I drove 46 miles, idled 27 minutes, then drove another 10 miles. The overwhelming raw gasoline odor and visible vapors again came from the fuel-filler area. A strong gasoline odor was also present underneath/around the driver's-side rear wheel well, but not around the passenger-side rear wheel. This recurring release of gasoline vapors presents a potential fire and occupant/public safety concern because gasoline vapors are flammable. I have kept the vehicle outside after incidents rather than parking it in an enclosed garage near an ignition source. The vehicle is available for inspection. The vehicle was previously inspected by an authorized Lexus dealer for this same concern, and Lexus Brand Engagement opened a case and technical-assistance ticket. The dealer reported no fuel leak and deemed the vehicle safe to drive but did not reproduce the condition. The vehicle was driven only approximately 3 miles at the dealership under different conditions. The problem has since recurred multiple times. There have been NO warning lights or dashboard messages. The repeated visible vapors and overwhelming raw gasoline odor during normal operation raise concerns about a potentially unsafe fuel/EVAP system malfunction.

We have had 2 windshields crack on the Lexus TX350, with very low impact to the windshield by very small road debris. Both windshields require(d) replacement due to long migrating cracks. There are several issues with the windshields on the TX350 per online searches. The windshields are a laminated acrylic one that are prone to cracks requiring replacement. This is a common issue on the TX350. The windshields are on long back orders for a genuine Lexus part. We are having our second windshield replaced on XXX at the Lexus dealer. I am of the opinion that this issue should be looked into by NHTSA for a potential recall and improved manufacturing process for future original equipment and genuine Lexus replacement. Our TX350 will be available for inspection on XXX at Parker Lexus in Little Rock Arkansas. The approximate cost to replace the windshield is $1850.00. We are not claiming the windshield on our insurance for this replacement for fear of potentially being cancelled by our insurance company. Please research this issue at your earliest convenience and feel free to request more information from me if needed to help in the assessment of the problem. We look forward to hearing from you asap. The electronic throttle control, vehicle speed control, and powertrain/transmission control system failed. When I press the pedal to accelerate there was no power. However I did not lose power to the vehicle as I can see the vehicle was in Drive mode but was not moving forward. I was on a decline hill and the decline helped me go downwards before a truck could hit me. This total loss of acceleration has occurred exactly 2 times within a 4-week period. The vehicle is currently located at the Lexus dealership service center and is available for immediate inspection upon request. My wife and small children were in the vehicle when the accelerator pedal went completely dead while driving. The sudden drop in speed in active traffic almost caused us to be catastrophically rear-ended by an 18-wheeler semi-truck. After one minute of zero throttle, the vehicle violently jerked, locked us into a highly dangerous 5 MPH limp mode on the road, jerked a second time, and then suddenly resumed normal speed. My family's lives were put at extreme risk. No. The vehicle is currently at the authorized Lexus dealership for inspection, but the service department states they cannot find any diagnostic trouble codes (DTCs) and have not yet reproduced the intermittent failure on a test drive. The vehicle is currently under inspection by the authorized Lexus dealership service department. It has not yet been inspected by the manufacturer's regional corporate specialists, police, or insurance. No. There were absolutely no warning lamps, check engine lights, dashboard messages, or symptoms prior to either of the two failures. The loss of acceleration happened instantly without warning. The vehicle's computer system failed to store any error codes despite the severe mechanical jerking and the sudden 5 MPH speed restriction.

We were driving down the highway and the vehicle started shaking. I immediately slowed down as I felt we had a flat. There was notification on the screen. I moved to the right onto the shoulder. Only after we were completely stopped did I see a low tire warning come on display. I slowly got out as the traffic was heavy. I could see the outer section of the tire came apart from both sides. As we were in a dangerous location someone called the police. Two police cars pulled behind us and provided us some safety. We agreed to swap out bad tire for spare as we were in a very dangerous location and needed to get vehicle moved before someone ran into all of us. We did look at the highway to ensure no objects were in the road. I didn’t see any while driving, but we wanted to verify to protect other drivers. There also weren’t any pot-holes in highway. We inspected tire after taking it off and there were no screws, nails, objects, or damage to the center tread/sides. The tire seemed to separate fairly cleanly on both sides. Tires were Continental Cross-contact 255/45 R22 107V LX20 (Eco plus) To repeat, no Warning Lights came on vehicle before tire came apart. I have sent message to my insurance company. No one has inspected tire and it is in my possession.

I am reporting two separate but related fuel-system defects on my 2025 Lexus TX500h (VIN [XXX]) that raise safety concerns and appear to be widespread across Lexus TX and NX hybrid models. FUEL FILLER DOOR FAILS TO OPEN: The electronic fuel filler door intermittently fails to open when the release is activated. It sometimes requires multiple attempts, and at times will not open at all, leaving the vehicle unable to be refueled. This creates a safety risk of being stranded with no ability to add fuel, including the risk of running out of fuel on the road or being unable to refuel in an emergency. The failure appears to worsen in cold weather. This is a widely reported problem on 2022-2025 Lexus NX and TX hybrid vehicles and has been attributed to a faulty electronic fuel-door lock actuator (referenced by owners and parts suppliers as OEM part number 77030-F6020). Owners are frequently forced to use a manual emergency release located behind an access panel in the cargo area in order to refuel. FUEL TANK CANNOT BE FILLED TO CAPACITY: On a near-empty tank, the fuel pump nozzle shuts off after only approximately 8 gallons, despite the tank's rated capacity of approximately 17 gallons. Adding additional fuel requires repeated, prolonged attempts and often only a few more gallons can be added. This significantly reduces usable driving range relative to what the vehicle's fuel gauge and range estimate imply, creating a risk of unexpectedly low fuel and reduced range in situations where refueling options are limited. Other owners have reported the same behavior, and in at least one documented case a Lexus Field Technical Specialist diagnosed the cause as a deformed/defective fuel tank requiring a redesigned replacement. Both conditions are present on my vehicle. I am reporting them so that they are documented at the federal level. No crash, injury, or fire has occurred. This failure is consistent with a documented, systemic design vulnerability in the Lexus TX platform. The vehicle's wide-open front grille design allows road debris direct access to the AC condenser, which sits as the frontmost component in the cooling stack with no protective barrier. Multiple NHTSA complaints document identical failures on TX platform vehicles at extremely low mileages including 580 miles and 900 miles, with dealers uniformly denying warranty coverage. Operating without AC in Texas summer heat constitutes a safety hazard, particularly for child passengers. I am requesting NHTSA investigate this as a systemic design defect pattern across the TX platform. Attached photos include: (1) photos provided by the dealer as their evidence of "impact damage" — none of which document a specific leak point or impact deformation zone; and (2) photos taken by the owner of the condenser on the Westside Lexus loaner vehicle provided during this service visit, which show comparable fin condition and debris accumulation to the denied vehicle. The loaner vehicle photos demonstrate that this condenser condition is consistent with normal operation in this environment, not discrete impact damage.

On our 2025 Lexus TX350, less than 1 year old, and under normal driving conditions, our AC stopped working suddenly. Upon inspection at the dealership, they determined a rock or road debris came through the front grill and impacted the condenser, which caused a major refrigerant leak and ac to stop working. I believe this was due to a poor design of the lower half of the front grill, it’s wide open and offers little to no protection to ac and engine components. They should have a better honeycomb or similar grill to protect components. The refrigerant leak is an environmental and road safety issue, and living in hot climates like southern Nevada, when it’s 110 degrees outside, the heat inside the vehicle with no AC is a safety issue for children, adults, etc. The dealership did not cover repairs under warranty, and quoted $3,500 to repair either with cash or submit insurance claim.

The load carrying capacity modification label may display inaccurate added weight values. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 110, "Tire Selection and Rims."
Consequence & remedy
Consequence: A vehicle with an incorrect maximum capacity weight value may be overloaded, which can increase the risk of a crash.

The rearview camera image may not display when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequence & remedy
Consequence: A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.
Remedy: Dealers will update the rearview camera software or replace the rearview camera, as necessary, free of charge. Owner notification letters were mailed May 15, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB02 and 26LA02.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequence & remedy
Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.
Consequence & remedy
Consequence: An instrument panel display that does not show critical information can increase the risk of a crash or injury.
Remedy: Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB08 and 25TA08. Lexus' numbers for this recall are 25LB05 and 25LA05.
